Your experience with Clinical Network typically starts with creating a new Request for Medical Records. In your life time, you probably have visited more than one doctor, hospital or a lab. You can now request and view your medical data stored with these providers. Keep in mind, that by law all medical providers are required to keep your historical data only for seven years. We will request to aggregate any of your available data if you so specified, but it may be limited to the last seven years only.
Once in the Dashboard, click green button Actions on the left menu and select New Request.
In the new window select one or more doctors, hospitals, or any other medical facility who you've visited in the past. If you can't remember all of them - no worries - we will start searching for your records also through our extensive medical records network.
You will need to select the type of data you are requesting. For example, you may want to aggregate all your medical data, but exclude results of an HIV test.
Once you selected your providers, on the next screen please read carefully and accept the User Agreement and sign your form (this is your HIPAA-proxy consent). We will then deliver it to the selected providers.
It is possible that some of your data will be available instantly; if not - we will send you a notification when it is available for you to view.
NOTE: Health Gorilla charges a small $1 processing fee per request. Some providers may request an additional processing fee before they release your records; we will notify of this and you will have an option to opt out. The provider's fee usually will not exceed $50 for really large historical files. Most providers, however, who use electronic medical records systems, will provide the documents without any additional fees.
